R.I.S.E. Integrated Mental Health uses MYIO as our secure patient portal.

MYIO is an important part of your care at R.I.S.E.

Please set up your account and download the MYIO app before your first appointment.*

Setting Up Your Account

1. Look for your MYIO Invitation

R.I.S.E. Integrated Mental Health will send you an invitation to set up your MYIO account by text message and/or email.

2. Download MYIO

Download the MYIO app using one of the links above, or access MYIO from your web browser.

3. Create your account

Open MYIO and select Create Account. Follow the prompts using the information provided in your invitation.

4. Complete your setup

Once you are logged in, please complete any forms, questionnaires, or other items assigned to you.

What Will I Use MYIO For?

You may use MYIO to:

Complete intake paperwork and questionnaires
View upcoming appointments
Join telehealth appointments and virtual groups
Receive appointment reminders
Send and receive secure messages
Complete forms assigned by your treatment team
Review statements and make payments

Please check MYIO regularly. Your R.I.S.E. care team may assign forms or questionnaires for you to complete before an appointment.

I never received an invitation.

First please check your junk mail or spam. If you still cannot locate it, please contact R.I.S.E. and we can resend your MYIO invitation.

I already have a MYIO account.

You do not need to create another account. Download the MYIO app and sign in using your existing account information.

I forgot my password.

Select Forgot Password from the MYIO login screen and follow the prompts to reset it.
