TMS (Transcranial Magnetic Stimulation) is a non-invasive, FDA-approved treatment for Major Depressive Disorder, Obsessive Compulsive Disorder (OCD) and other conditions. It uses magnetic pulses to stimulate specific areas of the brain that are underactive in depression.
TMS is typically recommended for individuals who have not achieved sufficient improvement with medication and/or therapy, or who cannot tolerate medication side effects. A thorough evaluation with your provider will determine if you are an appropriate candidate.
A full course of TMS for depression usually involves 36 sessions. Your first appointment will include a Mapping and your first treatment (45-60 min), followed by daily treatment sessions (Monday-Friday) for approx. 6 weeks. For the last few sessions, optionally, patients can taper their treatments reducing frequency of treatments over an additional 2–3 weeks.
Your very first appointment will include a process called mapping (process varies depending on what you are being treated for--detailed below). The mapping process ensures we are treating in the correct location every day. The mapping process may need to be repeated if you have any changes during treatment such as medications, substances, sleep, etc.
--For Depression: A single magnetic pulse is delivered to locate the correct treatment area. During this pulse, we will be watching your right hand for small movements. You may also feel mild muscle contractions on the right side of your face, such as eye closing or jaw tightening. This is completely normal.
--For OCD: A single magnetic pulse is delivered to locate the correct treatment area. During this pulse, we will be watching for a reaction in your feet. You may still feel the right side of your face tighten slightly, including eye or jaw muscles. This is also completely normal.
You’ll be seated in a comfortable chair. A cap will be placed on your head with a map on the front to ensure we are treating the correct location each day. A helmet or coil is then positioned over the treatment area. The device delivers magnetic pulses to a targeted brain region. You may hear clicking sounds and feel a tapping sensation on your scalp. You can read, listen to music, or relax during treatment. After your session, you are safe to resume regular activities, including driving.
Most patients experience only mild scalp discomfort or tapping sensations during treatment. These typically lessen over time. Some patients may experience mild headaches, which can be managed with over-the-counter medication.
Common side effects are temporary and may include: scalp discomfort or sensitivity, headaches, fatigue
Rare but serious side effect: seizure (very uncommon, ~1 in 200,000 treatments)
Clinical studies show that many patients experience significant symptom improvement, and some achieve full remission. Results may vary, and ongoing follow-up with your provider is important.
Yes. Most patients remain on their prescribed medications while receiving TMS. Major medication changes are typically discouraged while undergoing TMS. All medication changes should be discussed with your provider.
Not usually. TMS sessions take about 20-30 minutes, and you can drive yourself to and from appointments. You are safe to resume your normal daily routine immediately, including work, errands, and driving.
Consistency is key. Try to keep your schedule the same every day. Take your medications at the same time. Attend TMS sessions at the same time each day. Aim for consistent sleep (similar bedtime and wake-up time). Limit caffeine. We recommend keeping caffeine intake to 1–2 cups per day. Higher amounts can make your body more sensitive to TMS, which may increase discomfort during treatment. We also recommend attending our ReWiring Resilience group weekly while participating in TMS if your schedule allows.
